Laboratory Testing Consent and Release

What you agree to when you collect a sample at home and send it to the lab.

Self-collection

You collect your own sample using the guided kit provided. You agree to follow the instructions, including timing and handling, because collection technique affects results. If you are unable to collect a sample, contact us rather than submitting an inadequate one.

Testing and reporting

Your sample is analysed by a CLIA-certified laboratory. Results are released to the ordering physician and to you in the app. Some results are calculated rather than directly measured, and the panel notes which.

Limits of a panel

A blood panel is a snapshot. Results can be affected by hydration, recent exercise, illness, sleep, medication and the time of day. A single unexpected value is usually repeated before it is acted on. A panel is not a screen for every disease and does not replace age-appropriate screening with your own physician.

Critical results

If a result suggests an urgent problem, the laboratory and the reviewing physician follow their critical-result procedures and you will be contacted directly. Keep your contact details current. This is not a substitute for emergency care.
