Scope of this notice
This notice describes how protected health information (PHI) about you may be used and disclosed by the covered entities that provide your care — the partner physician network, and the pharmacies and laboratories involved — and how you can get access to it. AZZA Medical acts as a business associate of those entities and handles PHI under written agreement.
How your PHI may be used
For treatment — so a physician can review your labs and prescribe. For payment — to bill you and process your payment. For health care operations — quality review, training and administration. Where the law requires disclosure, such as public health reporting or a lawful court order.
Your rights
You have the right to inspect and copy your records; to request an amendment; to an accounting of certain disclosures; to request restrictions on use or disclosure; to request confidential communications; to be notified of a breach of unsecured PHI; and to receive a paper copy of this notice. Marketing uses and any sale of PHI require your written authorization, which you may revoke.
How to exercise a right or complain
Write to privacy@azzamedical.com. You may also complain to the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
AZZA Medical is not a medical provider and does not practice medicine. Medical services are provided exclusively by independently licensed physicians of our partner physician network and their affiliated professional entities.
Compounded drugs are not FDA-approved and have not been evaluated by FDA for safety or effectiveness.
This is not emergency care. AZZA Medical does not provide urgent or emergency medical services. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room.
