Authorization to Use or Disclose Health Information

The permission you give so your care team, lab and pharmacy can work together.

What you are authorising

That your health information may be used and disclosed among the parties delivering your care — the treating physician, the accredited laboratory, the licensed compounding pharmacy, and AZZA Medical acting as business associate — for the purpose of evaluating you, prescribing, dispensing, shipping and following up.

What it does not cover

It does not authorise marketing use of your health information or any sale of it. Those require a separate, specific written authorization that you are free to refuse without affecting your care.

Duration and revocation

This authorization lasts while you are a member and for the record retention period required by law. You may revoke it in writing to hello@azzamedical.com at any time; revocation is not retroactive to disclosures already made, and revoking it may mean care can no longer be delivered.


AZZA Medical is not a medical provider and does not practice medicine. Medical services are provided exclusively by independently licensed physicians of our partner physician network and their affiliated professional entities.

Compounded drugs are not FDA-approved and have not been evaluated by FDA for safety or effectiveness.

This is not emergency care. AZZA Medical does not provide urgent or emergency medical services. If you are experiencing a medical emergency, call 911 or go to your nearest emergency room.
