It is defined by variability, not by decline

The common picture — hormones gently falling year on year — is wrong. In the years before the final period, output becomes erratic. Some cycles produce more oestrogen than you have ever made; others produce very little. FSH rises, but it also swings. That is why symptoms can be severe in a month when the numbers look unremarkable, and why perimenopause is usually a clinical diagnosis made from the pattern rather than from one result.

What actually changes

Cycles shorten first, often by a few days, then become unpredictable. Sleep breaks in the small hours, frequently before anyone notices a hot flush. Mood and concentration shift. Weight redistributes toward the abdomen even when the number on the scale holds. Joints ache. Hair thins. Skin loses collagen faster in the few years around the final period than at any other time.

What to rule out before blaming the transition

Thyroid disease, iron deficiency, depression and obstructive sleep apnoea all produce the same list, all are more common in this age group, and all are treatable. This is why the AZZA panel measures TSH, free T4, free T3, TPO antibodies and a full iron study alongside the reproductive hormones. Treating the transition while missing a ferritin of 9 is the most common way this goes wrong.
