Before you blame your hormones, rule out the boring explanations

The four to exclude first

Obstructive sleep apnoea, which causes fatigue, low mood, low libido and can itself lower testosterone. Hypothyroidism. Iron deficiency, with or without anaemia, which is frequently missed in menstruating women and in endurance athletes. And depression, which is not a diagnosis of exclusion and does not become less real because a hormone is also borderline.

Why this order matters commercially

Every one of those four is cheaper to find and more effective to treat than a hormone protocol, and none of them generates a monthly subscription. A service with an incentive to prescribe has an incentive not to look. The honest version of this business model requires telling some people that what they need is a sleep study, not a protocol.

If you snore and you are tired, start there

Loud snoring, witnessed pauses in breathing, waking unrefreshed and daytime sleepiness are worth a sleep study before a hormone conversation. Treating apnoea often improves the same symptoms people arrive hoping a protocol will fix, and untreated apnoea is also a reason to be cautious with testosterone therapy.
