Estradiol in men is not the enemy

Crushing it is a common and avoidable mistake, and the symptoms of too little look a lot like the symptoms of too much.

What it does

Men produce estradiol by aromatising testosterone, and they need it. It contributes to bone density, libido, erectile function, lipid handling and mood. The popular framing of estradiol as a contaminant to be minimised during testosterone therapy is simply wrong, and acting on it causes harm.

The symptom trap

Low libido, fatigue, joint aches, low mood and poor erections appear when estradiol is too low as well as when it is too high. Someone feeling worse on therapy who assumes the cause is high estradiol, takes an aromatase inhibitor, and feels worse still has walked into this trap. The only way through it is a measurement before the assumption.

How it is handled properly

Measure it alongside testosterone. Where a level is genuinely high and symptomatic, the first lever is usually the testosterone dose and the route, not an aromatase inhibitor — and body fat matters, because adipose tissue is where much of the aromatisation happens. Blanket use of aromatase inhibitors to chase a low number is not good practice.
