How eligibility works
A 503A pharmacy may compound with a bulk drug substance only on defined grounds — broadly, where the substance is the subject of an applicable monograph, is a component of an approved drug, or appears on a list the FDA develops for this purpose. The agency evaluates nominated substances and publishes the outcome, including substances it has considered and decided should not be used.
The category that surprises people
A number of peptides that are extremely visible in advertising fall outside eligibility, and some have been explicitly identified as raising significant safety concerns for compounded use. For those, the issue is not whether the evidence is strong enough to justify trying — it is that no compliant pharmacy will make them, so no lawful supply exists. Anything reaching a consumer is coming from somewhere else.
The lists move
This is live regulation, not settled law. Substances are nominated, evaluated, added and removed, and guidance is updated. A claim about eligibility that was accurate two years ago may be wrong now, in either direction. That is a reason to ask your provider for their current position rather than relying on an article, including this one.
The one question to ask
"On what basis is this substance eligible for compounding, and which pharmacy is preparing it?" A provider operating properly answers immediately and specifically. Vagueness, a change of subject, or an appeal to how many people are already using it is the answer.
